This is a great idea for a thread!
Never had any trouble anywhere except for the Grand Hyatt in Singapore. Was trying to check into a club room and the guy asked at least 4 times as I don't think he thought I understood. Kept saying that the room they had for me wasn't suitable as it only had one bed, was quite funny but took him 5mins to agree to letting me keep the room.
Weird thing was he looked pretty camp lol

I fared a little better at the Grand Hyatt Singapore - but I think only because I flew in one day earlier than my partner. After he arrived, I did get some occasional looks of uncertainty, but nobody said a word.
I've had more obvious discomfort from the Mandarin Oriental Kuala Lumpur, where the in-room checkin agent actually had the nerve to ask if my guest was male or female! I told him male but he said nothing else and completed the checkin formalities. It was a junior exec suite I believe with one king bed.
I also was questioned don't I want 2 beds at the Westin Kuala Lumpur, but told them one bed was fine and no further argument. One security guy glared at me pretty openly, but nothing ever came from it.
I've never had a problem in any US, Australian or NZ lodging.

LAS VEGAS HILTON! I kid you not!!!
My spouse of 16 years and I booked into the Las Vegas Hilton for our anniversary and I have NEVER had so much more difficulty getting a King sized bed in my life. After many several questions going back and forth, we got the king room that I had requested. We've laughed about it many times since then.
We now live in Europe and I love visiting churches. My son loves to climb to the top of the steeples. Often there is a "family" rate. I love explaining in a Catholic church, that my partner, son and I should get a family rate. We've never been turned down either!

My partner and I have never really had any issues getting one king sized bed. No issues in the following hotels.

Never had a problem anywhere but then I only stay at mainly IC's/SPG properties outside of the US and some Hyatt's/Hilton's/Marriott's.
All they might ask if I prefer a King and that's it.
No problems with the local visitors either.

Hotels stayed in....
Had absolutely no problem in the following hotels:
Any UK/European hotels (Ireland, UK, Spain, Germany, Italy, Sweden, Belgium, Netherlands, etc) - both chains and independant.
Hilton Nairobi (got upgraded to a suite without asking, with one bed - offered a roller, but declined saying that we didn't want the staff to go to the effort).
Salt Lick Lodge (safari in Tsavo), no problem at all. Was on safari for a week and had a great time.
Dubai Creek Hilton - didn't book a specific room; allocated a king with a great view
Hyatt San Diego (upgraded to a suite)
Hilton New York
Waldorf Astoria
Roger Smith hotel, New York
Hilton San Francisco (upgraded to a suite each night - must be a perk
)
Hotel New York New York
Grand Hotel Europe in St Petersburg, Russia - was cautious but both greeted with a welcome drink, escorted to room and given a bottle of vodka and caviar as a gift. Made to feel more than welcome and felt very safe.
No problems with any airline either (although usually fly British Airways and they take care of you)

Our flight arrived at 3:40 am, and the three of us (my husband and a male friend) REALLY needed somewhere to sleep until our own 2 booked rooms became available. A hotel receptionist at one beachside hotel (next to Hotel de la Mer) rented us a room with one king bed for the three of us at 4:30 am for 4 hours. She gave us a quizzical look at first, but I guess she understood from our sleep-deprived looks that we just really needed to sleep.

I have travelled to over 100 countries, and usually with a partner in tow. There would simply be too many hotels to list for the seamless manner in which we were assigned a room with one bed....and never once has it been an issue, even in parts of the world where I expected some awkwardness.

We haven't had any problems anywhere except when we were playing tourist in our home town...San Francisco. I kid you not. The Fairmont desk clerk said they mistakenly booked us into a king. Not a mistake, we said. Oh, she said. Oh. Oh. I see.

The only place I can recall it being outwardly awkward (meaning it required verbal discussion) was the Sheraton Cairo in 2005.
Somewhat related -- upon checkout at the Hilton Tokyo a few years ago we were charged for two people in the room, even though HHonors members don't have to pay extra for a spouse or SS domestic partner. But the front desk didn't quite understand the same-sex thing and insisted on charging us for the extra person. Some rather comical attempts at explanation. I gave up and then just took it up with Hilton corporate, where it was satisfactorily resolved.

One place where we had an amazing experience was the Phinda Private Game Reserve in South Africa. From the planning stage via email to our welcome at the reserve, they welcomed us and took amazing care of my partner and I.
No issue with the bed, and they even created a romantic evening for us in our room, complete with the chef bringing us dinner.
Highly recommend this place, and they created an amazing environment for us (and some rooms have an outdoor shower, which is great as well!).

Only twice in 20+ years of travelling off and on with partners.
Once in a small Bavarian B&B in Augsberg in 1987, sorted after we explained we were gay.
Once at the Caledonian Hilton in Edinburgh in 2002, sorted by a manager being called and proceeding to issue a full scale dressing down to the trainee.
