The CDC website linked at the top says the order was rescinded two to three weeks ago.

CDC’s order requiring airlines and other aircraft operators to collect contact information for passengers before their arrival into the United States from a foreign country was rescinded on November 21, 2025.
I think it was set to expire end of 2025 anyway.

I had some one-offs with it being skipped over the past few years, but hopefully that means now the stupid form is actually gone!

Definitely one of the dumber things we had to go through, years after its necessity had obviously already expired.

I have always used my post office box, so this was never a beg deal for me.

I always just checked in at the counter, where they never asked. Maybe UA made something up.
