There are still some limited cases where you need to reconfirm your flights. By way of example:
- Aeroflot requires reconfirmation for destinations in Cuba, Africa, and India;
- British Airways requires reconfirmation for Libya and Saudi Arabia.

The reason to reconfirm flights to be aware of any last-minute schedule changes that may have occured whilst you were travelling and the airline would have to way to reach you. If they move the flight up and you miss it, they may say that you are responsible for not re-confirming. I had a flight routing change on the way back from Rome and without reconfirming, I would have been mighty confused, as that airport lines up by flight number rather than having a general airline counter.

Not to be confused with the requirement to reconfirm flights discussed above. If you are required to reconfirm the flight (e.g. at least 72 hours in advance) and fail to do so, the airline has a full right to cancel your booking (and resell your seat) even if the flight departs on schedule on date/time stated in your ticket and notwistanding the fact that you have the OK status on your ticket.
