The cheapest flight I have ever found is ZAL-ZCO (Valdivia, Chile to Temuco, Chile). Full fare on lanchile.com is 3,500 CLP (5.35 USD), plus tax minus 3% on-line booking discount, for a total of 5,200 CLP (7.95 USD).
That is so cool -- eight bucks total for a flight. Granted, it's only 78 miles (25 minutes block time), and there's only one flight a day, but that is a price that can't be beat.
Expedia.com says the price is $37 for a Y fare, but for some reason Expedia.com only has access to Y fares for all intra-Chile flights.
It might be an error (i.e., should be 35,000 CLP one way), but even if that's the case, it's cheaper than any other mistake fare that I know of when you include the taxes.

$16, can you get 1000 aa miles and q-points? That's only $800 for platinum.

Nope. The fare basis code is GOW, so assuming that's in the G booking class, you get no AA miles at all. You do get LanPass miles (kilometers, whatever). The cheap fares (super economica web) accrue neither AA nor LanPass miles (kilometers).

A couple of months ago, Air Asia was offering an non-refundable, non-changeable advance purchase fare for one of the domestic sectors in Thailand (I'm thinking Bangkok-Ubon Ratchathani but I could be wrong) - 9 baht (USD 0.225) each way. Add up taxes (I think it was 155 baht) and the grand total was something like USD 4.50

Ryanair have a lot of fares for 99p, however, that doesn't include taxes. I have also seen Ryanair list some flights as 'free' so that's not a bad deal. Shame the airports are so out of the way.
About 6 years ago I got a return flight DUB-STN on Ryanair for IRL0.02 (about $0.03) + tax. I can't remember what the taxes were but certainly nothing like they are now! Those were the days....

Not in the realm of "world's cheapest", but possibly up there in the annals of unusual fares: about 3 years ago (right at the tail end of TWA's existence), I booked a walk-up one-way fare MCI-STL-TPA for $62 all inclusive. I think the base fare was about $40. Just this year, I got a one-way ticket on Delta MCI-CVG-IND for something like $42 - a base fare of $27.
I do not believe that either was a "mistake" fare, however I don't believe anything is truly a "mistake" fare.
