I have a J award MXP-ZRH-NRT returning NRT-ADD-ATH mid March to early April for 2 people that I need to cancel.

I don't know if award availability will spring back once I cancel, but if anyone here is interested in this I think quite hard to find combo for 2 people, let me know in PM and we can give it a go.

Still high surcharges, I avoid redemptions with surcharge more than 250 euros.

I agree. Full fare would been over 3000 , so I am still happy with it
and to speak the truth, there were very few alternatives, even considering KL, SIN, BKK etc.

What would be the cost if you booked Economy cash fare and then upgrade with miles?

I am not sure if Aegean/*A follows this rule, but in QR if a flight is available for an award, it is also available for an upgrade.
This way, you get some extra miles/tier points and also avoid the surcharges.

As Fransknorge wrote, upgrades on Turkish are only available in classes Y and B. A quick search for random dates in February shows CDG-SGN-CDG available in B at EUR 2,840. The upgrade itself would cost 100,000 miles, and you would earn around 13,000 miles for the ticket. In addition, an upgrade would also trigger a recalculation of taxes and surcharges. Itamatrix indicates an increase of 334 in taxes, thus putting the total cost at 3,174 euros plus 87,000 miles.

Is this across all airlines when you use Aegean miles?
What if you are a Turkish airlines card holder? Does this still apply?

Nothing to do with holding a TK card. If you're using Miles&Bonus (Aegean) to upgrade, you're playing by the M&B rules. And according to them, it's usually only the 2-4 highest fares, that are upgradeable.
There actually was a TK promotion a while ago, where you'd get the B class for something like 800 EUR, so that would have been a great opportunity to try the upgrade via M&B... But in 99,9% of the cases it should be far more worth it to just issue your ticket in I, so a straight-away award ticket, instead of purchasing Eco and upgrading.

The upgrade to F may actually be rather interesting for longer flights. At 65K for most regions (From EU) thats kind-of a steal if you can find a reasonably well priced economy ticket thats eligible.

Unfortunately, the *A upgrade to F is from C (again, booking classes vary by airline), so while it may still be decent value, it may not be as good of a deal as if you could upgrade from Y.
