Which island ?
If you were back-packing in Thailand for the first time with your family which included a couple of young teenage boys and wanted an island you could reach by a train and ferry journey from Bangkok and which wasn't full of tourists but had just enough to keep the kids enthralled and amused which ones would you choose.
Koh Lanta has been mentioned to me but any others ?

I'd say Koh Phi Phi Don. Spectacular beaches. But it may have more tourists than you care for.
Although, I don't see the specific need for an Island. I would probably recommend Railay Bay in Krabi, were it not for your specific request for an Island.

No there's no specific need for an island so any recommendations for anywhere in Thailand that's easily reachable from BKK would be much appreciated.
I just don't want somewhere that's heavily touristy or sleazy or full of ageing hippies wearing spandex and getting stoned on a beach, if you know what I mean.

I'm not as familiar with the islands as many others, however I've had great times in off seasons. Samui can be great in June (some beaches at least). I've heard good things about Kata in Phuket before the rains come too.

I'm not looking for a specific resort or area just something ad hoc where we can spend a few days and then move on.
I quite like travelling to places by train as well.
I also have absolutely no knowledge of Thailand whatsoever so you'll have to excuse my ignorance.

Sure looks like it. Please start by consulting a map, and you'll find that your question is absurd, as the mainland has the same climate and same appearance as any islands next to that part of the mainland.

Well, bearing in mind I've got two young teenage boys all activities are pretty much geared to them.
So I suppose water sports, snorkelling, elephant trekking etc would be on the list but not in a hugely touristy area - the kids are happy travelling ad hoc and don't require full resort facilities.
And it would be nice to see some authenticity which is really why I asked whether mainland might be better than island.
Oh and Mrs Showbizguru wants to sleep at least one night in a hut over the water.
And I quite like sitting in a beach bar throwing back cocktails while watching the sun go down.
And everything has to be quite cheap even though it's December.

I still think that Railay Beach in Krabi is the best choice - even though it's not an island - for all intents and purposes, the experience is the same, because it's isolated from the rest of the mainland by large rock formations, and needs to be reached by boat.
