I understand there is a single shower in the Amex Centurion Lounge. Holders of run-of-the-mill Amex card holders can access that lounge for USD 50. Its free for Centurion and Platinum Amex card holders.
I've read in the relevant thread over in the Amex forum, that there is often a lineup for the shower. :-( Why they would build this with a single shower I dunno. |

The Club SFO in domestic Terminal 1 (Harvey Milk) has showers. They take Priority Pass. Potential problem is they sometimes have a waitlist to get in.

The Club charges $25 to use the shower for 30 minutes. No clue about the cleanliness or water pressure. Google Reviews mention towels not being replaced or taken away in between uses, which leads me to believe they don't clean the showers too often.
The Centurion Lounge shower has really low water pressure (their sinks too), as part of a "water saving" program. (I've complained at the front desk a few times.) It may be an airport-wide initiative, so The Club's showers may also be low, too. |
