That's exactly what I was just about to say. :)

Several months ago, I went to a US Passport Agency in person because I needed my passport immediately.

I showed up without an appointment and explained my situation to the passport agency clerk. She was very helpful and accepted my application and fees(an extra $60 for expedited, as I recall) That was around 11AM. She told me to go have lunch and come back at 1:15PM. I came back at 1:15PM and my spanking-new passport was waiting for me. I took a quick glace at the data page and everything seemed fine. I thanked her and left.

When I got into my car, I looked at the data page of the passport again. To my horror, I discovered that they somehow managed to reverse my first and last name. (my first and middle name became my last name, while my last name became my first name) I went back into the agency and showed her what they had done... She apologized and told me to have a seat. About 20 minutes later, she came out with another passport with the error corrected. :)
