I just pulled this together for the folks I work with, obviously if you have an account with any of these airlines, it's smart to go ahead and sign up now, even if you have nothing planned (I've always been a bit of an airline tramp, so I fly and accumulate miles in all these programs myself!):

This was great. Could we add the bonus EQMs from partner offers to the original post and make it a definitive post or sticky for all EQMs?

UA added a TRIPLE option in response to DL:

Offer: Earn a total of triple Elite Qualifying Miles* (EQM) on qualifying United flights in United First® (F, A, P), United Business® (C, D, Z) and full-fare United Economy® (Y, B).

It is also possible to earn EQM with the United Visa Platinum card (annual fee $140). Not sure if other airline affinity cards offer EQM.

Looks like ffI has a good list above. I would add that first time United Visa Platinum CC users will also receive 1 EQM for each $ charged to United.com.

You are right The first year is 15k, next yr is 10k max.

Earning EQM: Upon enrollment in the program, you will receive a one-time bonus of 5,000 EQM. Does anyone know of any other Tier Miles Bonus Programs being offered by any of the airlines now, or in the not too distant future, especially Delta or Northwest?

AS DEQM is good through June 30 (though the better transcon fares are long gone- at one point, a DCA-LAX-SEA-PDX-ANC r/t could net you some serious mileage for fairly cheap, when adding in an elite RDM bonus). I have had luck getting flights credited on AS metal even when the ticketing carrier isn't AS (in my case, AA), so it's possible you could find a lower fare on AA, NW, DL or CO if you looked. I doubt that any flight not on AS/QX metal will credit DEQM, though.

AS status does provide domestic upgrades/elite seat selection on NW (and supposedly will carry over to DL in 2010- it's probably equivalent to DL Silver Medallion, without the Skyteam elite status), as well as on AS, and you can earn full AS EQM on AS/QX, AA, NW, DL, KL, AF, and LA, as well as RDM on BA, QF, CX and CO (and some other smaller airlines).

AS is also pretty nice about status matches, FWIW.
