I remember reading somewhere its against their TOS or something.

Well, I don't do it. Their terms say:

"Cash back is not offered on fee-free promotional codes."

Yet, some have reported that they still get cash back. But I don't choose to push my luck. I take the free shipping and pay the $3.95.

It has prevented me from getting CB

I use a code that lowers the shipping to $3.95 and keeps the $3 fee and get CB

I haven't done it, but I'm pretty sure it's once per Amex card. I plan to cancel before 90 days and start up again with another Amex on the account.

The whole program seems to be targeting people who buy them on behalf of others - like a corporate purchasing department. They reward the people in the department via BigCrumbs and this $25 card.

I got this email too from my Costco Amex but since several have posted that if you don't pay the fee you don't get BC cash back then it would be worthless would it not? Or am I missing something?
I have had to file a missing credit with BC for a legit purchase so I know I for one am not going to chance not earning the CB to save a few bucks.

Well, some have said they got CB even with a code. So some take a chance and it works. It wouldn't be the first time that the terms don't agree with reality.

But I feel the way you do: not worth losing the war (1.4%) and winning the battle ($3.95).

How do you guys exit out of Amex GC since there's no PIN for BB load or for MO purchases. I can only think of AP to exit or actual spend. Any other ways?

You need to do two steps at a minimum, not just set a code. YMMV as to when/where that works.
