One of my best sushi experiences was in Vancouver. The key is of course knowing the chef (itamae), but also picking a slow day, and show up early. It was omakase, and for the next hour he fed me perfect pieces of sushi at well timed intervals. Granted, the rice was probably American, but the rice portion, density, amount of vinegar, and temperature were all perfect. The matsutake broth was the way it should be. Perhaps not Kyubei, but very close. Can't remember how much I paid, but definitely Vancouver prices.

Good quality sushi can be found outside of Japan, even in places where they serve California rolls if you order right. Hong Kong comes to mind.

As for Shanghai, I remember going to a decent sushi bar years ago in the Westin (now Grand Sheraton), close to the Japanese expat community. But the Chinese market seems to prefer generalists with encyclopaedic menus (even in HK). Japanese specialist restaurants (sushi, unagi, katsu) tend to have a hard time abroad.
