Trip to Spa in Utah - 1x - 9/30 dl - NO AIR
A 4 day/3 night trip for two to the Red Mountain Spa in St. George, Utah and a one year supply of Tampax Fresh. Requires filling out a survey about tampon usage.

Entries must be received between June 28t, 2005, 12:00 am EST and September 30, 2005 11:59 pm EST. Entries will be deemed to have been submitted by the authorized account holder of the e-mail address submitted at the time of the entry. “Authorized account holder” is defined as the natural person who is assigned to an e-mail address by an Internet access provider, online service provider or organization (e.g., business, educational institution, etc.) that is responsible for assigning e-mail addresses for the domain associated with the submitted e-mail address. Sweepstakes is open to all legal residents of the continental United States and Canada (except Quebec) who have reached majority age in their state of residence at time of entry, except employees of Gruner + Jahr USA Publishing (“Sponsor”), its parent, subsidiary and affiliate companies, its participating advertising and promotional agencies, and the immediate families of the foregoing employees. Submission of entry constitutes permission for Gruner + Jahr USA to use entrant’s email address and other submitted information for purposes of marketing and promotion. Sweepstakes is subject to all federal, state and local laws and regulations. Void where prohibited or restricted by law. Entrants agree to be bound by the terms of these official rules and by the decisions of Sponsor, which are final and binding on all matters pertaining to this Sweepstakes.
Selection of Winners. Total number of eligible entries received determines odds of winning. From all eligible entries received, Fitness Magazine will randomly select one (1) Grand Prize Winner and two (2) First Prize Winners on or about October 3, 2005. Winners will be notified by email, mail, or phone on or about October 3, 2005. Winner must reply with all contact information requested in the notification within ten (10) days from the date the first attempted notification was sent, or another winner may be selected. Winner may be required to sign an affidavit of eligibility and liability/publicity releases (unless prohibited by law) to be returned within ten (10) days from the date it was sent, or another winner may be selected.
Prizes: Grand Prize (1): Grand Prize Winner will receive a Spa Weekend Gift Package, which includes hotel accommodations at Red Mountain Spa in St. George, UT for four (4) days and three (3) nights and $100 gift certificate for spa services for each of the 2 people and a 1-year supply of Tampax Fresh® (Six boxes, with each box containing 36 tampons). The approximate retail value of the Grand Prize is $1,000. First Prize (2): Two First Prize Winners will each receive a 1-year supply of Tampax Pearl® or Tampax Compak® (Six boxes per winner, with each box containing 36 tampons). Approximate retail value of First Prize is $50. All expenses not specified herein (including, but not limited to, air and ground transportation), and all taxes are the sole responsibility of the Winner. Selection of hotel accommodations and spa services, including, but not limited to spa location, amount and kind of spa services at Sponsor’s sole discretion. Spa Package must be redeemed by March 31st, 2006. Certain black out dates may apply for Spa Package. Travel companion of Grand Prize Winner must also execute a Publicity/Liability Release prior to ticketing and must possess and show Sponsor valid photo I.D. Once the dates for the Spa Weekend Gift Package have been arranged, the dates cannot be altered and failure of Winner to follow such schedule shall not obligate Sponsor in any way to provide the Winner with alternate arrangements. No transfer or substitution of prizes except at Sponsor’s sole discretion. Sponsor reserves the right to substitute prize of equal or greater value if prize is unavailable. The prize is awarded without warranty of any kind from Sponsor, express or implied, without limitation.
Limitations of Liability/Reserved Rights. Neither Sponsor nor its parent, subsidiary or affiliate companies, nor its promotional agencies shall have any obligation or responsibility with regard to (i) entries that contain inaccurate information or do not comply with these rules, (ii) entries, prize claims or notifications that are lost, late, incomplete, illegible, unintelligible, damaged or otherwise not received by the intended recipient, in whole or in part, due to computer or technical error of any kind, (iii) telephone, electronic, hardware, software, network, Internet or computer malfunctions, failures or difficulties, or (iv) any damages or losses of any kind caused by participation in this Sweepstakes or resulting from acceptance, possession or use of any prize. Sponsor, in its sole discretion, reserve the right to disqualify any person tampering with the entry process or the operation of the Web site or otherwise violating these rules. Sponsor further reserves the right to cancel, terminate or modify the Sweepstakes if the Sweepstakes cannot be completed as planned because of infection by computer virus, bugs, tampering, unauthorized intervention or technical failures of any sort.
