Answered it. A few comments, if you can edit the questionnaire without messing up the results:
1. Answers to the question "Have you used miles for non-flight awards?" end with "seldom." It should have "never" as a possible answer.
2. The term "round trip segments" is confusing. A flight segment is one takeoff followed (one hopes) by one landing. A round trip, by definition, includes at least two of those.
3. The last two questions, if you think your miles will increase/decrease in value over the next two years, are the same question written two ways. Are you looking for framing effects here? If not, delete one of them. (If you are looking for framing effects, separate them by some other questions on other topics to make their connection less immediately obvious.)
