Not necessarily.

There's a video somewhere on Youtube (and elsewhere), IIRC, it was made by a German to demonstrate how much an overweight person could conceal just in rolls of fat. It was a very effective demonstration of the limitations of the NoS in scanning very obese people. IIRC, he was, in part, countering the arguments of some that the surrender position would 'stretch' fat rolls and raise breasts so far that anything concealed under them would fall out or be exposed.

Or is TSA suggesting a 'part 2' scan where the pax bends over, spreads legs and grabs ankles, to dislodge/expose anything concealed between his/her cheeks?

I've seen many individuals, of both genders, including quite a few in blue uniforms, who could have successfully concealed fairly sizable items between their butt cheeks (or in their fat rolls).

Of course, if it's metallic, the WTMD would probably catch...but WTMD is no longer primary...

Very inconsistent. About a month ago, at SNA, I forgot that I had my wallet in my front pocket. It is a big wallet, over 1.5" thick. No anamolies on the screen.

That's just part of the "unpredictability" layer of security the TSA talks about. Will the scanner find the object or not? Will it false alarm on sweat or nothing at all? :D

Maybe they thought you were just happy to see them and be patted down?

Physics isn't your strong suit, is it? Or were you meaning in the same sense that "BDO work is based on Science! (tm)" In which case I would agree.

Not only is the WTMD not primary, but when I opt out I am not even allowed to go through it. Not once in an opt out have I gone through a metal detector - I am always directed to go around it and through the gate.

In my younger days I could not have sneaked anything through by hiding it under my breasts. But now... well, suffice it to say if I were a bad person there is plenty of space to conceal something... large. :D And, based on the patdowns I receive, TSA would never find it.
