The only exception I can think of is the transfer of miles from a deceased relative. Earlier this year I had AA transfer miles under these circumstances and, subsequent to faxing a death certificate, only my RDM's increased by the RDM's from a lifetime Gold's account. Which makes sense.

So, that would be 40K miles at the end, correct?

Or is it realy 6:1 now?

The HH to miles transfer is very low. It has certainly been this ratio for the past three years at least

10,000 AA miles = 20,000 HH points
10,000 HH points = 1500 AA miles.
