It happened to me last year when take off was delayed due to luggage loads, I was in business class and chatted a bit with the captain. He comes back a few minutes later telling me he saw I was one of their best customers (platinum for life and platinum seem to be differentiated on their ipad) and since it was his birthday, he would be glad that I join them for the flight in the cockpit. It was an excellent flight

I know for a fact that the UK does not allow it any more for any flight. I did think that Israel was the same, except that another poster gave evidence of flights where passengers have been in the cockpit to/from Israel.

Sorry man but apart from the exorbitant sexism in your post, sounds more like to me that the woman in heels was non-rev & airline employee and since the flight was full, she took a seat in the cockpit? Don't see any drama here.
On the sidenote, sterile cockpit is a rule that applies for landing and take off (i.e. when the plane is below 10000 feet), not during the entire flight (when autopilot has full control of the flight anyway).
