If I remember correctly, it had to be cleared by a manager and the manager would refuse unless you actually needed to use the card. I suppose you could say you wanted to use it and get it done, but otherwise they weren't interested in helping.

I got fed up of their attitude and at that time, it used to be a marathon getting them to acknowledge that 'double exposure' had happened. They know about it alright and they probably know how to fix it too but it is easier this way as I was told once, not many people keep an eye and notice and it drops of anyway after a bit

It doesn't happen with any non AMEX card I use and the airlines the same so it must be AMEX's issue. They used to like to blame the airlines too I remember
