Boarding Paperwork for infant/lapchild

Can anyone who's traveled with infant/lapchild recently tell me what the counter hands out as a boarding document for a lapchild? Is there a separate boarding pass with the adult's number on it? Or is it just a record notation with no separate boarding pass (and how does TSA handle it)?
