Given a similar situation I just called and requested awards in my own name using the deceased account. You may even book on line with some programs. The way I understand the rules of the program miles are not transferable upon death. Honestly I never informed the airline. Nor would I now if you can simply issue the awards you want on line without informing the airline of the situation.
