Although I had not had a problem on all of my previous trips, the last time we returned from Mexico (after my wife warned me not to) I again ate the salad on my meal tray. About an hour later (fortunately we had already landed), I became violently ill and remained so for the next six hours. Then I was fine again. I am sure that such an occurrence is pretty rare, but it was so unpleasant that I won't chance it again. The same advice holds in Mexico unless you are eating at a nicer restaurant which takes precautions to protect our sensitive tummies!
A bit off-topic, but if you go down there much - look into Dukoral. Its an oral cholera vaccine that also protects against E. Coli. Two doses. Not approved in the US, but about $100 for 2 doses from Canada from a reputable canadian pharmacy (beware scams). I used to ALWAYS get sick when I left the US, but the Dukoral reduced this to very occasionally. And when I do get sick, the veracity of the symptoms is reduced.

I can even have ice in Mexico City if I feel like it, and probably be ok. That would have been impossible before Dukoral.
