1) United doesn't charge a change fee on award tickets as long as the routing doesn't change. Would a stopover be considered a routing change?
2) I understand that departure taxes for Narita are normally added to the airfare. How would the additional departure taxes be paid.

I guess one way around around this issue is to add the stopover in the original ticket and pay the departure taxes and then delete the change if necessary. This could get expensive though on "stopovers" in London or other cities with expensive departure taxes.

Yes, UA allows the change. Adding a stopover requires a change fee. Deleting a stopover does not. If all flights are on UA metal changes can be made to the itinerary after departure. The UA CSR I talked to said you could be credited the departure tax if you delete the stopover.
