MLL lounges - when are they occupied to capacity?

I have never been to any lounge (MLL or otherwise) filled to capacity until June 3, when I was turned away at the UAs RCC lounge in concourse F at ORD. The lounge at B6 that day was essentially full, but I and others were let in.

The question is which MLL lounges get full, and during what (peak) flying hours? If a MLL lounge becomes full, does the MLL stop all passengers from coming in, or the lower tier passengers and/or those who are flying in the economy cabin? (PS-I have never been turned away by AC's MLL lounge, or CP's Empress lounge).
