When I received my bonus for in-flight assistance (also am MD), it took a good 6-8 weeks after I sent back the letter for the miles to show up. Just in case you thought it was fast.

I don't remember what the exact numbers were, but I had a choice between 10-15 K status miles and 20-25 K regular miles. I chose the latter...
