The only thing I can think about is not making a qualifying flight in a three year period. But since you wrote you have been flying regularly since 2002 that cannot be it.
Do you still have e-mail or paper statements from you Flying Blue account? And still they do not want to give your miles back?
I had something similar with Emirates. Wrote to customer service and the next day everything was repaired!
Anyway good luck dealing with Flying Blue... I hope you manage!
