These mistakes can happen when all upgrades are not confirmed in advance and an agent has to intervene.. or when there are last minute changes to your itinerary due to standby or irregular operations. It's happened to me recently as well..but only in those cases. It's always been a good idea to check after each flight to be sure the correct number of instruments were taken.
