Sorry, don't have any pictures, but from what I remember Japan Airlines & Philippine Airlines both had them on their 747s.

The beds on JL were more private than those on PR as the dividers between the beds were higher, on PR the dividers were lower and the upper deck F/A could keep an eye on the cabin and its occupants. I think because of this, the PR beds were also certified for passenger occupancy during take-offs and landings and this was mentioned in their print ads. On JL a seat on the lower deck was required for take-off and landing.

I read something interesting back in the 1980's that the female F/A's on JL were complaining about working the upper deck because the businessmen up there were asking for massages.
