The phone/airport agents do that quite often... it happened to me on my last flight (I booked with paper vouchers, so it was at the airport). I didn't get any error messages, although I also didn't try to upgrade... but FWIW, I had zero problems at check-in and the miles posted just fine despite the apparent name difference.
I've had this happen as well. Instead of Ms. Go Places, my name always shows up as "PLACES/GOMS" on boarding passess from itineraries booked online. I keep waiting to be stopped by a TSA agent looking for "GOMS" on my ID, but so far, so good.
