The credit card companies pay for the miles with funds earned from merchants (generally equal across all cards of a brand), annual fees to cardholders (generally higher for award cards), and interest earned on revolving balances (interest rates are generally higher on award cards).

So, who pays? You pay. The only question is: do you get enough miles to make up for what you pay? Obviously some people do, and some people don't.
