1. Whatever lounge IB contracts with at IAD. There may not be any. IB web site just says lounge is available "at major airports" and does not itemize which ones. BA does have a lounge at IAD which IB could use (for a fee), or you could use with OW status (which you don't have).

3. no arrivals lounge at MAD (or any other airport, for that matter). Only BA/QF/CX/AA have arrival lounges at LHR (plus a few lounges outside of security that can be used for arrival), otherwise most airlines do not offer arrival lounges. MAD could have done so as the new terminal was built from scratch, so it is a conscious decision by IB not to have arrivals lounges.
