1. SAS
SK's new A330 and A340 planes do have a section called "Economy Extra". This is similar to BA's "World Traveller Plus", BD's "New Economy" or VS's "Premium Economy", in that it is treated as a separate cabin, with some slight enhancements to service, seat width, etc.
It is not generally treated like E+ (the only way in is to pay full fare Y) though I have heard of some Star Gold members getting upgraded at the gate (and I would imagine that SK Star Golds would have little problem getting a seat in Economy Extra).
No mileage plus upgrades available on SK, so I guess its down to asking for a discreitonary upgrade on the day!
2. LH
As Rudi correctly pointed out, no LH plane anywhere has E+ or anything resembling it. Asking for an exit row sometimes works though (the seatmaps on the LH website and in their paper timetable help here).
LH upgrades using UA miles (and vice versa) are a bit bureaucratic, in that you have to apply to mileage plus for an LH paper upgrade voucher, which they mail to you. I have done this in the past but have been able to get upgrades confirmed over the phone, as long as I am able to produce the voucher at check in. Not really that bad, as long as you plan a few weeks ahead. If the upgrade doesn't clear for some reason, you can apparently return the voucher to UA for a full mileage refund.
