This has happened to me on a couple of occassions. I spoke to the US Embassy in London and they said it was in my interest to send a copy of my passport front page and the slip with a covering letter as it could cause issues next time I tried to enter the US.

I doubt that I would have had any issues but have always decided to err on the side of caution - just in case!
