I have the same problem with a trip in June 2007 on SAS. Sent in all original boarding passes and detailed receipt from AMEX, showing the ticket numbers.
UA won't budge because the boarding passes don't show year. Give up with AMEX, they can't help you.
Here is UA told me to do to get credit; you may want to try it.
UA said no credit without a copy of the original ticket. You need to contact Air New Zealand for a copy of your ticket if you don't have it. I was able to get another copy of mine from SAS and it took
only three months. This copy needs to be sent back to UA Mileage Plus, along with the following:
Date ticket was issued
Passenger name
Cities of Travel
The operating carrier
The flight numbers
The class of service purchased
Dates of travel (including the year of travel)
The amount paid for the ticket
The ticket number
Good luck. I just sent all mine back in yet again and am waiting to see if it finally works.
