This is what I've been told, and have experienced, as well. If my companion and I are not upgraded 5-6 days out, then I have never received the upgrade for us both. On two occasions, I was upgraded at the gate, but my companion was not.
(Quoting myself...) Just experienced this again -- received gate upgrades to/from my recent destination and both times was told that I had the last seat and no FC space was available for my companion. On the return flight, I was told I was #9 on the waitlist (75 mins before flight) and only 1 FC seat was open, but then received the last-seat upgrade at the gate. No other gate upgrades were handed out.
I really think that Platinum + Companion is <<< than Platinum in terms of the upgrade order/priority. I think I am about 2/10 on successful platinum companion upgrades, and both were on short hops.
