A quick look at the Hamburg Airport web site reveals lots of info on transport in the region: http://www.airport.de/en/verkehrsanbindung.html

There appear to be shuttle/transfer services to Bremen or probably a train via the Hamburg Hauptbahnhof (search timetable here). There might also be some info over in the Germany forum.
