But ... isn't the clear trend for new airport capacity to position it - where practicable - away from cities, or to avoid approaches over cities? Off the top of my very empty head, Lisbon and Athens, and Madrid's new runway system, are European examples this enlightened approach.

The decision to aim well outside the city is probably fed by opportunity cost considerations regarding land-use. Pretty pertinent to the Heathrow question I'd have thought.
