I do remember one occasion, on a domestic flight, where, as SE, I did not get my choice.
When ordered, they took it. However, since I was in the back, somehow there was a mismatch and when they came to me, there was only the god forbid chicken left.
The other issue: I often fly with my wife on an Aeroplan reward. When they come to ask me, I direct them to her also right away. So far so good, but I have the feeling that there is some resistance/policy agaisnt taking her order together with mine. Which is at the very least awkward.
Mind you, given the poor quality of kaiseki food out of NRT lately, might have been better otherwise. Oh well...
