IT's a manual takeoff, and depending on the pilot, manually flown up to at least 10,000 ft. In the 767/757 we can turn the autopilot on at 200ft, but thats not normal procedure. Some guys like to fly it all the way up to cruise altitude, some only up to a few thousand feet (personally, I fly it at least up to 10k).

For landing, depending on weather conditions, the autopilot is usually disconnected at some point during the final approach. If the weather is down to minimums we'll usually leave the autopilot on until we see the runway. The 767/757 is certified for autoland, but thats not normal procedure for us. However, we are required to do at least 1 autoland per calendar month.
