I use my neck strap as a shoulder strap, then wrap it around my hand when shooting in certain situations.
Being mugged can happen no matter what. You have a camera out anywhere, it is a risk you take, so you that can't really be in the front of your mind. I have worked in some of the worst places around, including spending months at a time shooting a story on narcotics enforcement in one of the worst projects in Brooklyn, New York, at the height of the crack epidemic and never got mugged.
In any environment you need to be constantly scanning your surroundings when you have cameras out. I travel with two on my shoulders, out, in plain view, almost everywhere I travel.
