The one other factor I would add to LIHPrem's list in determining whether or not to upgrade is when you're going with regard to school schedules. If your trip is post June 15 or so you can pretty much expect lots of children in the back. Not that kids aren't great, it can just get a little noisy back there on some of these summer vacation flights. I always upgrade my SFO-Hawaii flights.
