The OP forgot the most iimportant part: the additional S&H fee.
The TMO prepaid sims on eBay seem to price out best at $6 (delivered) for up to 60 days and 150 minnutes. That's $0.04/minute domestic.
It works great if you have an unlocked or TMO locked GSM phone with 1900 and possibly 850 MHz.
The phone number and area code can usually be changed to one of your choice. TMO customer service is usually great.
If you like the service, the number and balance can be extended another year with a $100 refill (can be bought for $89 ($0.089/minute) if you shop the internet) and subsequest years at $10 each.
Some people keep churning the $6 sims but they never get to keep the same number that way.
I like TMO prepaid so much that I fund it for myself, 2 kids, mother, father-in-law, and retired neighbor (they use it to/from HI a lot). For low volume users, MUCH cheaper than any contract.
Downsides: No international roaming, no free nights/weekends, poor or no ural coverage in some parts of the country when away from interstate highways.
For me, using TMO prepaid for my USA/domestic cell usage and local prepaid sime in other countries works great.
