In my one experience flying UA 3, I also noticed the cabin isn't cleaned at OGG. When I boarded, I needed to wipe down my center console because it was sticky. They required all ORD passengers (except those requiring wheelchair assistance) to deplane in KOA so the plane could be cleaned, which was annoying because KOA is such a small terminal and there wasn't really anywhere to sit. Seems like this was different from your experience.

As far as blankets go, my experience is that on the Hawaii flights, similar to domestic F, the blankets aren't replaced after each flight. An easy way to tell this is whether the blanket is wrapped or not. I don't think the "freshness" of blankets is any different on UA 3 vs. other Hawaii flights.
