FWIW, I just call them and instruct the agent to book me in V class so I get the 100% EQM - my travel budget is set for the year and there is a specific amount for each trip. If my air ticket costs abit extra to get the full mileage, I just trim expenses on other items, like hotel/car/food so I balance out. I still never get close to my budget limit anyway.
It's a hassle - and the only time I got what I thought was a less-than-dumb-as-a-rock Amex agent to ticket a co.com reservation I made online (at a much lower fare than Amex could find), it took her 30 minutes on a conference call with CO support to do the ticketing, then after the call, she clicked 'cancel' on the website and totally messed up my trip.
I hate Amex travel - they are beyond useless.
Not only does AMEX have trouble finding the lowest fares on airlines, wait until you get to your AM EX booked hotel. Good luck with AmEx you'll need it.
