I think the "does not apply to upgrades" means upgrades for operational reasons.
I recently (27 December) flew MAN-CDG-SIN in Business/First (Business class ticket upgraded with miles) and got access to the First Class lounge and the "private car" (actually a mini-van shared with other passengers) service. However, there was no personalized ground service, but I did get escorted to the First class lounge after I asked about its location in the Business class lounge that I managed to locate
My "escort" said the personalized ground service was currently only for passengers starting their trip at CDG, but that AF was working on implementing it for transfer passengers.
I think you can also get treatments in the Clarins spa, but it was closed when I was there for an evening flight.
