I've been on flights when additional fuel is requested & loaded seeminglly at the last minute. What changed in the calculations that the fuel wasn't calculated correctly the first time?
I'd say most of the time, the weather has gone down at the destination and you need an alternate now, or the alternate weather has gone down and you need an additional alternate or now there is weather enroute you need to get around or now there is holding at the destination and you need holding fuel..etc...
What I'm getting at is there are numerous reasons why fuel might be added at the last minute. Mostly weather related.
