As nacirema stated, there are no set rules. I purchased two tickets this morning: one was a straightforward point-to-point award on BA and the other was a very complex international, multi-city "paid" ticket on AA. The former was ticketed (and an e-ticket sent to me) within minutes, while I have still not received the AA ticket, which, mind you, departs in less than 5 hours.
