In the UK, the requirements are:

• Otherwise able to enter the UK.
• 2 entries in the last 6 months or 4 in the last year, to show you actually need IRIS.

There is no paperwork to fill out... just show them your documentation. They'll scan you, check that it properly identifies you, and then you can start using it the very next time you enter the country. It works very well for me.
