I would expect you to manage the practice of your profession in a manner that is prudent to your patients' well-being and honors your obligation to prudently consider the impact to those affected by your actions and decisions in whatever state of mind or body you may be in.
If you can handle getting off a long flight and jumping into the OR without messing anything up, then do it.
If you know you can't, then re-schedule as you need to.
If you chronically can't, and consistently lose patients (both literally and figuratively), then you would need to evaluate whether you're managing your schedule properly, in the right profession, etc.
