I would contact Customer Relations at US Airways and tell them you do NOT want a voucher, you want RESOLUTION and/or a refund.
I would file a complaint with the DOT and your state's consumer protection agency.
It's likely that Customer Relations won't be able to help. They deal purely with past travel issues regarding US. Did the OP book through a travel agency? It seems so, as the company who cancelled the reservation is nothing to do with US. They need to call the issuing agency, it seems to me.
