I figure if there was some sort of requirement for documentation, an SQ GA would definitely have asked for it. Without it, the dog wouldn't have gotten on a plane. They are known for going by the book, after all.
I'm really not sure how the FAs would have handled it, once the dog used the bathroom on the carpet.
