This topic is interesting since I have been given an extraordinary amount of difficulty in getting credit for miles flown(in January) and the problem according to MP staff is that the ticket had my 1st and last name and my MP card says my middle and last name.
I provided MP with copy of ticket and stubs of boarding passes for segments claimed and they still want further proof(although I sent this info certified mail they would not acknowledge receiving it even when I have proof of delivery!!!).
Ironically, my wife got the miles and they know we travelled together...

The skies may be friendly but not at Mileage Plus...!
